How to Take the Quiz:
- For each statement, rate yourself on a scale of 1 to 10, where:
- 1 = Almost Never (this is rarely true for me)
- 10 = Almost Always (this is almost always true for me)
- Be honest — this is for your benefit. There are no right or wrong answers.

- The quiz should take less than 10 minutes to complete.
